  • Lifestyle changes, memory care shape Alzheimer’s support | Long Island Business News

    With approximately one in eight elderly Long Islanders diagnosed with Alzheimer’s disease, treatments for this progressively debilitating disease are essential.

    And as the population ages, yet even more people will be diagnosed with dementia and/or Alzheimer’s and require memory care services either at home or in a group living facility, making services and treatment for people with cognitive impairment more vital than ever.

    Providing health insurance coverage for 3 million New Yorkers, EmblemHealth continually strives to bring evidence-based, holistic care to the communities it serves, notes Daniel Knecht, the company’s chief medical officer.

    Currently, EmblemHealth is partnering with Dr. Dean Ornish, a lifestyle medicine pioneer, and CookUnity, which prepares home-delivered meal kits. The pilot program—which follows Ornish’s research demonstrating improved health outcomes with intensive lifestyle changes—is designed for those diagnosed with early Alzheimer’s disease.

    According to Knecht, Ornish has found that diet, exercise, stress management and social connectedness–when adhered to by patients with early stage Alzheimer’s or memory loss—significantly changes the trajectory of the condition. Further, Ornish has done clinical studies that show that people on this kind of diet have reduced symptoms of memory loss, experience less of a foggy mind and have more energy through the day.

    Knecht added that the U.S. POINTER Study (conducted by the Alzheimer’s Association), demonstrated that a similar approach to lifestyle changes can reduce the risk of developing Alzheimer’s in the high-risk population.

    For several years, the medical community viewed Alzheimer’s as a fixed, chronic and ultimately fatal condition, not one impacted by lifestyle, notes Knecht .

    “We’re starting to really understand that it [Alzheimer’s] is a chronic condition akin to diabetes or cardiovascular disease where there are many drivers that can cause or worsen Alzheimer’s,” he shared.

    Knecht added that studies have shown that up to 45 percent of Alzheimer’s cases are avoidable by embracing a healthy lifestyle.

    “This is incredibly groundbreaking to have a health plan to advance access to a holistic lifestyle program for cognitive health. And we’re also using our clinics and our neighborhood care centers to bring this program to life,” he said. “We’re hopeful that the information and data will demonstrate efficacy to a point where we’ll just cover this more broadly.”

    Another aspect of Emblem Health’s pilot program entails exercise, Tai chi, mindfulness and meditation–activities offered at their 15 neighborhood care centers, many of them in underserved communities, according to Knecht.

    Because food is highly personal and has an important cultural aspect to it, the company wanted to “make sure we were bringing culturally relevant, delicious food that certainly aligns with the guidelines Dr. Ornish has set out and top quality,” Knecht said. “That’s where CookUnity fits in.”

    According to Bruno Didier, head of CookUnity Business, the pilot program— which is geared for up to 150 patients—is set to commence on Oct. 22.

    He says it will be a plant-based diet, which will consist of three daily meals, absent of sugar, salt, oil, dairy and meat.

    “It was a very interesting challenge for  the chefs to come up with recipes that would adhere to the diet,” Didier added.

     

    Memory care ‘neighborhood’

    A 20-bed memory care “neighborhood,” The Grove is the newest part of Jefferson’s Ferry’s life plan community.

    Set up in a circular pattern, The Grove is a ground floor unit where residents live in a more homelike setting, with access to a courtyard and an open kitchen. Group programming includes music and art, cooking classes, pet therapy, aromatherapy and virtual reality.

    “Grove activities are specifically tailored to the interests and abilities of the residents, including smaller, more intimate activities that encourage greater opportunities for meaningful and engaged participation.” said Anthony Comerford, vice president of health services/administrator for the South Setauket facility.

    To ensure the safety of residents, The Grove is a standalone unit where all activities are conducted within the neighborhood, according to Comerford.

    “An enclosed garden provides an additional safe space for enjoying outdoor activities. such as gardening, or for just enjoying fresh air and sunshine,” Comerford said. “The residents love spending time outdoors.”

     

    Secure unit and community programs

    Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park has a secured, high acuity 42-bed memory care unit for those who have been diagnosed with Alzheimer’s or dementia.

    “The programming is tailored to people with dementia, and we’re able to successfully maintain people when, unfortunately, they’ve had a diagnosis of dementia,” said Michael Rosenblut, Parker Jewish president
    and CEO.

    Rosenblut adds that for the most part, residents “may have Alzheimer’s or dementia and another related illness.”

    The patients, who are medically managed by physicians, nurses, social workers and nurse’s aides, receive specialized programming through Parker’s recreational department.

    Parker Jewish also operates the “Willing Hearts Helpful Hands” community-based program where people caring for their loved ones at home can get relief outside of their homes at memory cafes with live entertainment in Queens, Nassau and Suffolk counties. Since its inception in 2016, the program has engaged with almost 11,000 caregivers and their care recipients throughout Long Island.

    “We have one patient from years ago who participated in that program, I’ll always remember the wife said to us, she hadn’t been out with her husband—the last family wedding was 20 years earlier—even though he has dementia, she hadn’t been dancing with him in 20 years, and now she was dancing with him,” Rosenblut said.

    In July, Parker Jewish introduced another community-based program—GUIDE (Guiding an Improved  Dementia Experience), which offers caregiver support, education and respite, medication management, and home visits and assessments.

    Inspired Healthcare Capital, a private equity firm specializing in senior housing investments, has fully subscribed Inspired Senior Living of Hamilton DST, a Delaware statutory trust offering that owns a 195-unit Class A senior housing property in Hamilton, New Jersey. 

    The DST offering launched in early May 2022 and raised more than $56 million in equity from accredited investors through a network of independent broker-dealers and registered investment advisors. Proceeds from the offering, with leverage, were deployed to purchase the senior housing property for $115.3 million. 

    Located approximately one hour from Philadelphia and New York City, the four-story property was built in 2017 and encompasses independent living, assisted living, and memory care. Situated on 23 acres of land, it consists of studio, companion, and one- and two-bedroom units with a total of 204 beds. The company noted that the property is the only full-continuum community within 15 miles and the only provider of independent living in the greater Hamilton region. 

    “We are very pleased to continue to offer highly sought-after senior housing real estate opportunities to financial advisors and their investors. This will be our fifth fully subscribed DST offering this year with another eight DSTs coming out in the next 45 days,” said Patrick Lam, President of Capital Markets. 

    Inspired Senior Living of Hamilton DST offering was structured to generate investor distributions at an annualized rate of 6.25%, the company said. 

    Inspired Healthcare Capital LLC is an alternative investment sponsor based in Scottsdale, Arizona, that focuses on senior housing real estate with more than $800 million in assets under management. IHC raised approximately $60 million in May 2022 and is on target to raise approximately $600 million in 2022. IHC currently has 50 active selling agreements and relationships with over 28 broker dealers. 

    COVID-19 Despite the difficult lending environment created by COVID-19, Inspired Healthcare Capital was able to secure financing on multiple Senior Housing acquisitions as well as honor and maintain all distributions to investors in 2021, whereas other sponsor firms reduced or suspended distributions. During this time, IHC closed on nine properties worth $163,350,000 and was able to secure financing of $42,730,000.

    Discovery Senior Living (“Discovery”) had 34 of its communities earn top spots in U.S. News & World Report’s inaugural Best Senior Living rankings, which were published on May 10. Aimed at helping seniors and their families to identify and choose among the highest-quality senior living communities and care provider options in their respective areas, the Best Senior Living guide is a 2022 debut offering from U.S. News & World Report, itself the foremost, global authority in health ratings and consumer advice, attracting over 50 million visitors per month. 

    The initiative began last fall, and used objective, independent statistical assessment of consumer satisfaction survey data gathered from thousands of participating Independent Living, Assisted Living, and Memory Care communities throughout the United States.

    Dr. Mosley, professor and neurological expert will share the latest advances in Alzheimer’s and dementia research happening now at the University Of Mississippi Medical Center’s (UMMC) MIND Center. Attendees also will have the opportunity to take a virtual look at the new Gertrude C. Ford MIND Research Center in UMMC’s state-of-the-art Translational Research Center.

    The MIND—Memory Impairment and Neurodegenerative Dementia—Center is one of the country’s leading research and clinical care services working to crack the code of Alzheimer’s disease. By combining pioneering research, population-based studies and more than 30 years of data with brain imaging and genetic technologies, the clinical team is making discoveries about Alzheimer’s and its risk factors. The disease affects 5 million Americans and more than 40 million people worldwide.

    The mission of the Gertrude C. Ford MIND Research Center is to uncover the underlying causes of Alzheimer’s and related dementias and to develop new treatments. Since the center opened, researchers have not only advanced the understanding of how the brain ages but also identified multiple factors that predict the development of dementia. Uncovering causes and risk factors can lead to improved management and possibly even prevention of cognitive decline with age.

    Dr. Mosley is the MIND Center director. A professor of medicine-geriatrics, he has an extensive background in clinical neuropsychology, behavioral neurology and population-based studies in neuroepidemiology. His research efforts have been largely devoted to uncovering how and why the brain changes with age and his contributions have been recognized with several awards. His work has led to discoveries about the brain that show that cognitive decline begins much earlier than previously thought—an insight that could translate into earlier detection and treatment. His latest work promises to push the frontiers of science and medicine by bringing together state-of-the-art brain imaging and powerful new genetic technologies.

The continuing care retirement community (CCRC) offers two facilities for memory care: Campbell Cove and the Hughes Center. Both are licensed by the State of Mississippi as “Alzheimer’s Assisted Living” facilities and together, they offer a warm, home-like environment for individuals facing memory loss.

    The architecture at Campbell Cove and the Hughes Center follow design principles to minimize obstacles so that residents can maintain their independence as long as possible. For example, buildings feature wayfinding cues and circulation paths to guide people from one place to another. They also have small yet distinctive neighborhoods that allow for individual privacy while supporting family-size gatherings.

    Campbell Cove accommodates 36 individuals in three color-coded wings, each with a common living room and dining area along with 12 furnished private bedrooms. In the center is a large multipurpose area, arts and crafts room, recreation center, and kitchen. A secure walking garden and landscaped patios give residents the opportunity to spend time outdoors in a familiar and protected setting.

    The Hughes Center was created for individuals with advanced memory challenges and offers a calming ambiance for small-group sensory-stimulating activities. Each of the two distinct neighborhoods has 12 furnished private bedrooms and a common living room, dining room, kitchen, and screened-in porch and covered patio.

    Beyond the architecture, programming at the two facilities is person-centered with scheduled—but also unplanned—activities to enhance each resident’s quality of life. The staff follows a social model of care to support each person’s capabilities and affirm his or her dignity.

    St. Catherine’s Village offers skilled nursing dedicated to memory care, as well. A total of 24 private bedrooms with in-room showers are available for this living option, which combines person-centered memory care with skilled nursing services.

    Unparalleled services and facilities are the hallmark of St. Catherine’s Village, as is companionship, a caring staff, access to on-site resident-centered nursing care, and a mission-focused environment.     Those who move into either the Hughes Center or Campbell Cove, which offers a home-like environment for individuals facing memory loss due to Alzheimer’s disease and other forms of dementia, can save $1,000 per month for their first three months at St. Catherine’s Village. A limited number of rooms are available at monthly rates for individuals who are not enrolled in the community’s all-inclusive Life Care program.

    In the spirit of giving, we have reduced the short-term cost to move into St. Catherine’s Village and receive comfort, care and compassion.

    For people who need 24-hour care, the $3,000 fall special also is available at Siena Center. The skilled nursing accommodations include private and semi-private rooms. Siena Center was twice voted the best nursing home in Mississippi by readers of the Clarion-Ledger.

    Residents at the Weston Assisted Living Residence, part of the Lester Senior Housing Community in Whippany, once again enjoyed a festive week as they observed National Assisted Living Week. The annual event took place this year from Sept. 11-15; this year’s theme was “Family is Forever,” which acknowledged and celebrated the many ways we make family in our communities.

    Seniors and their loved ones kicked off the week with a special family photography session at a wine & cheese reception; visitors also toured the community’s new Memory Care Suite and learned more about the specialized memory care now available there. Other activities during the week included sharing good cheer and good times at a musical cocktail hour and expressing their inner artists at a creative arts program facilitated by Community Life Coordinator, Keisha McDonald. The week concluded with a discussion about the meaning and importance of family, as residents shared stories and insights. Afterwards, residents created a Weston family tree which showed everyone anchored as a family by the roots of the Lester Senior Housing Community, with each person branching off according to their varied interests and experiences.
