    study published in Nature Molecular Psychiatry is being used to question the use of SSRIs for people with depression. The study involved a new umbrella review of past studies of depression’s relationship with serotonin levels. One major conclusion is that there is no convincing evidence that low serotonin levels are the primary cause of depression. While the review has made headlines for “debunking” the serotonin imbalance theory, it is important not to jump to conclusions on the efficacy of antidepressants, particularly selective serotonin reuptake inhibitors (SSRIs). 

    This new study has led to various articles that have focused on how “scientists have called into question the widespread use of antidepressants.”  However, this does not mean that doctors will stop utilizing antidepressants as a treatment option. Instead, it calls for more research about why antidepressants work the way they do.

    Srijan Sen, M.D., Ph.D., is concerned about the impact of a new study about the role of serotonin in depression that’s getting a lot of attention.

    Sen notes that mental health experts certainly don’t believe that a simple ‘chemical imbalance’ is the root cause of depression.

    Serotonin is one of the primary brain chemicals, called neurotransmitters, that helps brain cells “talk” to one another by connecting to receptors on cells’ outer surfaces.

    The new study that’s getting lots of attention looks at a lot of older studies about serotonin, and tries to draw conclusions by combining the information from them.

    The study did not conduct new experiments or even combine previous studies in a meta-analysis. But instead, the researchers conducted an “umbrella review” of some, but not all, meta-analyses related to serotonin.

    By coincidence, another such “study of studies” on serotonin and depression was published just a week before the one that’s in the news. It concluded that serotonin transporter gene variations do play a key role in depression risk, in combination with stressful experiences across a person’s life. But that study hasn’t gotten nearly as much attention.

    In other words, Sen says, the science of serotonin’s exact role is far from settled.

    An article by Ruairi J Mackenzie in Technology Networks explains how this study doesn’t quite debunk years of science, since most scientists don’t adhere to the “chemical imbalance” theory on depression in the first place.

    The review article, published by an international research team including first author Prof. Joanna Moncrieff, aimed to assess the available evidence for and against the serotonin theory of depression systematically. The team explain this theory near the start of their paper: “[The theory is] the idea that depression is the result of abnormalities in brain chemicals, particularly serotonin (5-hydroxytryptamine or 5-HT).” The theory has been around for decades, but their overarching conclusion is that it is not correct, given that there appears to be no link between measurable serotonin concentration and depression.

    The reaction of many academics to this finding can be summed up, briefly, as “Obviously!” In comments to the UK-based Science Media Centre, Dr. Michael Bloomfield, consultant psychiatrist and head of the translational psychiatry research group at University College London (UCL), said, “The findings from this umbrella review are really unsurprising. Depression has lots of different symptoms and I don’t think I’ve met any serious scientists or psychiatrists who think that all causes of depression are caused by a simple chemical imbalance in serotonin.”

    Criticism of the review call into question how these indirect measures of serotonin function or are merely proxies for serotonin activity. They also question how depression is defined since there are multiple factors that contribute to the cause.*

    What remains possible is that for some people with certain types of depression, that changes in the serotonin system may be contributing to their symptoms. The problem with this review is that it isn’t able to answer that question because it has lumped together depression as if it is a single disorder, which from a biological perspective does not make any sense.

    “Many of us know that taking paracetamol can be helpful for headaches and I don’t think anyone believes that headaches are caused by not enough paracetamol in the brain. The same logic applies to depression and medicines used to treat depression. There is consistent evidence that antidepressant medicines can be helpful in the treatment of depression and can be life-saving. Antidepressant medicines are one type of treatment alongside other types of treatment like psychotherapy (talking therapy). Patients must have access to evidence-based treatments for depression and anyone taking any treatment for depression who is contemplating stopping treatment should discuss this with their doctor first.”

    With over 460 million global cases thus far, 6 million of which have resulted in death, the COVID-19 pandemic from SARS-CoV-2 proved to be a dire health crisis on a global scale. In this concerning scenario, the unprecedented effort by the international scientific community is even more remarkable and led – in record time – to the deployment of mRNA and viral vector-based vaccines. With more than 10 billion total vaccine doses administered worldwidevaccination campaigns have already started to attenuate this global crisis. In order to maximize vaccine effectiveness, the efficacy and duration of protective immunity will need to be systematically assessed and monitored as widely as possible.

    A team of internationally renowned medical scientists recently published a related paper in Nature Biotechnology, addressing the efficacy and sensitivity of a new type of assay to detect SARS-CoV-2 cellular immunity.

    “To date, antibody tests have been the typical, if not the only clinical endpoints commonly used to measure the immune response to SARS-CoV-2,” explains Antonio Bertoletti, Professor at Duke-NUS Medical School. “Both the humoral (antibodies) and cellular immune response act in coordination to achieve long-term protection from viral infections. Antibodies are important for preventing infection, protecting the body from disease, while cellular immunity is essential to eliminate virus-infected cells, thus helping to fight the disease itself,” concludes Professor Bertoletti. 

    Therefore, according to the study, measuring the T-cells’ response is a new disruptive approach to fighting SARS-CoV-2. An individual negative to an antibody test could still be protected thanks to responsive T-cells.

    This new study thus shows the relevance of a quantitative PCR approach to T-cell testing, thanks to the joint effort with SYNLAB, one of the world’s leading providers of laboratory diagnostic services, and Hyris, a global biotech company focused on AI-powered genetic analysis.

    “The assays used in our study rely on the quantification of CXCL10 mRNA after incubation of whole blood with SARS-CoV-2 specific peptides. The levels of induced CXCL10 transcripts correlate robustly with the IFN-gamma produced by activated antigen-specific T cells, serving as a proxy to detect cellular immunity in COVID-19 recovered and SARS-CoV-2 vaccinated subjects,” points out Ernesto Guccione, PhD, Professor of Oncological Sciences, and Pharmacological Sciences, at Icahn Mount Sinai.

    This is particularly important after the emergence of SARS-CoV-2 variants like Omicron that evade most of the neutralizing ability of antibodies, but not that of T-cells,” continues Megan Schwarz, a graduate student at Icahn School Mount Sinai and first author of the manuscript. “Precise measurement of cellular responses underlying virus protection, therefore, represents a crucial parameter of immune defence.”

    “The solutions already present on the market are usually performed based on the traditional fluorescence methodologies typically used in the immunological domain,” adds Jordi Ochando, PhD, Assistant Professor of Oncological Sciences, Medicine (Nephrology), and Pathology, Molecular, and Cell-Based Medicine at Icahn Mount Sinai. “This research demonstrates that CXCL10 mRNA expression confirms data obtained with traditional methods (i.e., ELISpot) compared with naïve, COVID-19 convalescent and SARS-CoV-2 vaccinated subject,” he concludes.

    This study could prove a key solution to further support the scientific community playing a strategic role in facing this global challenge.

    SinuSonic, a brand of Healthy Humming, LLC, is pleased to announce the presentation of the results from their study on “Double-blind, sham-controlled trial of a novel device for the treatment of viral upper respiratory tract infection.” This study was discussed during an oral podium presentation at the American Rhinological Society Spring meeting in Dallas, Texas, on April 28-29, 2022. This study showed, with the highest level of evidence, a randomized sham-controlled study that regular use of the active SinuSonic device improved nasal congestion.

    The prospective study was conducted at the Medical University of South Carolina in 2020-2021. Administration of acoustic vibration and oscillating expiratory positive pressure with SinuSonic has been shown in a prior study to improve nasal congestion and air flow. These interventions are hypothesized to release nasal nitric oxide, a molecule with known antiviral properties. The current study investigated the use of this device to prevent viral upper respiratory infections (URI) and reduce the severity and duration of rhinologic symptoms. 

    Asymptomatic community-dwelling adults were randomized to receive an active or a sham device (3:1). Subjects used the assigned device twice daily beginning at the start of the fall URI season. A validated metric of viral URI symptoms, Total Symptoms Score (TSS), was assessed each day for 8 weeks.

    Topline outcomes:

    • Those using the active device had 70% more days with no nasal congestion (57.2% vs 33.5%, p= 0.033)
    • A statistically significant difference in nasal congestion score was seen between the active and sham groups (0.503 vs. 0.843, = 0.036)
    • No subject in either the active or sham group developed symptoms meeting the study definition of a viral URI, likely due to viral precautions during the pandemic.
    • No major adverse events were detected, with 97.5% of subjects reporting zero pain or discomfort at the study conclusion.

    Amina and Nusrat Jamal, MD residents, have been following the COVID-19 pandemic since it became a national concern in March of 2020. They were deeply alarmed about the shortages of masks and other PPE, especially the predicament the healthcare workers were finding themselves in, such as reusing PPE that is intended to be single-use. In order to alleviate the pressure on healthcare infrastructure, they decided to donate one million medical-grade (N95) face masks to help state agencies and healthcare providers in the DC Metro area during the ongoing COVID-19 pandemic. The Jamals tried to purchase the masks but the manufacture and supply of masks were stretched thin worldwide. As a result, they decided to set about manufacturing the masks themselves and switched tracks to the purchase of N95 mask-making equipment and raw material. 

    Mr. Jamal, a migrant from Pakistan, is the CEO of Shireen Inc, a Germantown, MD-based manufacturer of wireless equipment. He believes in giving back to the community and the country which has given him so much. Silverline Health, a company established by him to aid in his healthcare endeavor, will be producing the masks in the USA. “I created Silverline Heath in response to the crucial need for these masks in the community and country. The goal is to manufacture and distribute these masks so they can reach our community as safely and quickly as possible, as the spread of COVID continues to grow.” The company is also beginning the arduous process of obtaining OSHA certification for these masks, ensuring that the PPE meets or exceeds standards that are being relaxed due to the emergency nature of the pandemic.

    Flow’s face shields are constructed robustly for multiple use and cleaning primarily of high-grade Polyethylene Terephthalate modified with Glycol (PETG), a material commonly used in the manufacture of FDA-approved face shields, along with hypoallergenic foam padding that rests against the user’s forehead and an elastic band for “one size fits all” simplicity and comfort.

    Vision Help Foundation is playing a central role in helping the citizens of Haiti become better prepared for the COVID-19 Coronavirus pandemic. The St. Louis based organization has gone deep into remote areas to reach individuals and families who might otherwise not have access to accurate information and health supplies.

    Local Haitian volunteers assisted in distributing COVID-19 Preparedness brochures door to door throughout the countryside with more than 2,000 brochures placed in the first week.

    Representatives also appeared on local radio along with information available from the Vision Help Foundation website and social media accounts.

    “Our volunteers went to local churches to tell groups how they can stay well, safe, and resist the virus. We also sponsored talks using a loudspeaker in public areas,” said Keller Polynice and Velly Duverny, VHF founders.

    In some regions of Haiti where water and sanitation supplies are scarce, Vision Help Foundation distributed buckets with soap, detergent, and safe water for people to use. This helps address public places where there are large groups of people with no sanitation.

    Vision Help Foundation is currently looking for funding from interested individuals and organizations. This will help more people properly prepare for COVID-19 both overseas and in the US. VHF is hoping to buy virus test kits and much needed ventilators. They welcome health care professionals who wish to volunteer.

    “The virus knows no boundaries or borders. It can quickly jump from one nation to another. It’s vital we help people in poor areas prepare so they don’t become hotbeds of infection. VHF is grateful for the generous support we have received from donors and volunteers. We need more of both to continue this important work,” Polynice said.

    VHF also conducts campaigns to improve public awareness of cancer, diabetes, and HIV/AIDS. They plan to offer small loans to help women start their own businesses and assist orphans in affording school.

For nearly 20 years, Dr. Vera has taken on a variety of roles in medical education, medical communications, corporate training and strategic operations that provide her with the diverse background that will serve as a critical foundation to lead Spire’s future transformation. Dr. Vera joins Spire with a reputation for driving strategic initiatives through positive individual, departmental and organizational growth. 

    Dr. Vera’s most recent role was as Global Head, Global Medical Affairs Training at Takeda Pharmaceuticals International, Inc., where she led business initiatives to enhance the scientific, functional and professional capabilities of the medical affairs organization. Theresa holds a Doctor of Philosophy (Ph.D.) in Pharmacology and a Bachelor of Science in Chemistry. “Spire Learning has been known for quality medical education programming for over 10 years. I look forward to collaborating with the Spire team to drive continued excellence in healthcare professional education. As healthcare treatment approaches become increasingly complex, we have an opportunity to be at the forefront in supporting professionals to prepare for the increased complexity of care,”  Vera said about her new role with Spire.

    Catherine Besthoff, Director of Risk Management and Patient Safety, Mount Sinai West, NY

    Catherine M. Besthoff, DrPH, RN, CPHQ

    Catherine Besthoff is the Director of Risk Management and Patient Safety at Mount Sinai West in New York City. Her expertise in health systems delivery has contributed to multiple patient safety campaigns and clinical quality improvement initiatives statewide and nationally.

    Catherine holds a public health doctorate in health policy and management from the CUNY School of Public Health and Health Policy. Her doctoral research focused on the patient and family engaged care in addressing hospital safety concerns. She served as an Expert Panel Member to the Nursing Alliance of Quality Care on Fostering Successful Patient and Family Engagement: Nursing’s Critical Role, and to the American Academy of Ambulatory Care Nursing Ambulatory Nurse Sensitive Indicator Task Force.

    “I am delighted to join Pulse Center for Patient Safety Education and Advocacy and extend patient- and family-engaged care into the community,” says Besthoff. “Patient safety knowledge is needed at home before you, a family member or friend visits a doctor’s office or becomes a patient. Pulse proactively provides this support and serves as a lifeline when it is needed the most.”

    Mary A. Horton, MSN/ED, RN

    Mary Horton is an Assistant Professor and Faculty Educator in the Department of Nursing at San Antonio College, San Antonio Texas. She is an avid and passionate champion for health care quality; committed to ensuring a safe practice environment that promotes the best, evidence-based patient care outcomes.

    Mrs. Horton’s clinical experience of over 45 years as a registered nurse has prepared her as a nursing mentor and role model to experienced and new to practice graduate nurses. Her master’s in nursing specialty is nursing education. She is currently completing final requirements for her Doctor of Nursing Practice degree in Educational Leadership. Mrs. Horton serves as vice president of the Theta Eta, Inc. Chapter of Chi Eta Phi Sorority, Inc., (a professional nursing sorority) and is involved in numerous community efforts to educate individuals, especially the at-risk and disadvantaged population, about health promotion and disease prevention practices. Although she has always been an advocate for quality and safe care, a personal experience of the loss of her only sibling during an elective procedure prompted even more dedication and personal service to help improve healthcare delivery.

    Pulse Center for Patient Safety Education & Advocacy (formerly PULSE of New York) is a grassroots, nonprofit, 501(c)(3) organization working to improve patient safety through advocacy education and support services. Our advocacy services are always provided free. Pulse also offers expert professional speakers to medical organizations and community/civic groups, and we seek sponsoring venues to host our training programs.

    Stropoli, who retired as Chief Financial Officer of Munich Health NA, a healthcare finance company, is now a business management adviser. He is a certified mentor with the Small Business Administration, mentoring university students and early career business personnel.

    Gary holds a BBA in Finance and Business Administration from Hofstra University and an MBA with Graduate Honors from Regis University. He is also licensed by the New York State Department of Financial Services as an Agent in Accident/Life & Health.

    “I recognize the significant impact of patient safety on the entire healthcare system, from patients and their families, to providers of healthcare services and the payers in the healthcare system as well,” Stropoli explains. 

    Gary’s passion for excellence in healthcare, his drive to inform and educate people to help them achieve better healthcare outcomes, and his personal experiences with the challenges of patient safety have prompted him to join Pulse in the pursuit of its mission.
