  • Green slime, mold, roaches found at Dallas restaurants; 1 closed for low score

    Two Dallas restaurants failed during the Jan. 18-31 round of health inspections, and another establishment was temporarily closed after receiving a low score, according to data from the city compiled by the Star-Telegram.

    Dallas food establishments are scored on a 100-point system that is different from Fort Worth. A range of 100-90 is considered very good. Anything under 60 is unacceptable and requires closure and inspection before reopening.

    Ceviche Loco Seafood Restaurant at 7861 Lake June Road scored 55 on Jan. 29 and was temporarily closed. The inspector noted fruit flies throughout the kitchen, overflowing floor drains, heavily dented cans and employees who failed to wash their hands.

    Local Tap and Table at 3333 Harry Hines Blvd. received a failing score of 65 on Jan. 31. The inspector found mildew inside the ice machine, nearly two-week old chicken stored in one of the coolers and mold on boxes in one of the storage areas.

    Pho Midway at 17610 Midway Road also failed the inspection with a score of 67 on Jan. 22. Shrimp was held at an unsafe temperature, food debris and black dirt were found inside the ice machine and a pot of food was stored on the floor.

    All failing restaurants require a follow-up inspection within 10 days. Establishments that score between 79 and 70 pass but also require a follow-up inspection.

    Fireside Chicken And Tacos at 18110 Midway Road scored 70 on Jan. 23. The inspector found moldy tomatoes in the cooler, green slime on the ice chute and some foods without a date marking.

    Scrambler Cafe at 18208 Preston Road scored 71 on Jan. 21. Raw eggs were left at room temperature, some cans were badly dented and there was food debris on the can opener. The inspector also observed a roach underneath the grill.

    Taqueria Express at 1909 S. Cesar Chavez Blvd. scored 71 on Jan. 22. Raw beef was stored over raw vegetables, the ice machine was dirty and there was no thermometer in the reach-in cooler.

    Faye’s Fusion at 1011 Corinth St. scored 72 on Jan. 21. Raw meat was stored at an unsafe temperature, some food was stored on the floor and the vent hood filter system was dirty.

    Feng Shui at 2355 Olive St. scored 72 on Jan. 22. The inspector noted pink mildew in the ice machine, fruit flies in the bar and chicken at an unsafe temperature.

    Shin Chon Market and Foodcourt at 11422 Harry Hines Blvd. scored 76 on Jan. 29. Raw meat was stored above the vegetables in the freezer, some employee food was stored with the establishment’s food and the back storage area needed to be cleaned.

    Roaches were found at two other establishments in addition to Scrambler Cafe.

    Maroche’s Bakery and Party at 1227 W. Davis St. scored 83 on Jan. 22. There was no certified food manager on site during the inspection and a dead roach was found in the storage area.

    Rise Southern Biscuits at 2414 Victory Park Lane scored 85 on Jan. 21. One of the coolers was leaking and two roaches were seen next to the dishwasher.

A score of 100 is a perfect score and 70 is considered to be extremely poor. Scores below 80 require a re-inspection, and below 60 require closure and inspection before reopening.   • These were the worst Triangle restaurant sanitation scores reported in 2025

    In North Carolina, state law requires restaurants undergo between one and four inspections per year.

    Sanitation scores and their corresponding letter grades are used to assess restaurants’ adherence to rules and standards intended to mitigate and prevent the spread of food-borne illnesses, as explained in The News & Observer’s weekly roundup of restaurant sanitation scores.

    In each inspection, restaurants start with a score of 100%, but points are deducted with violations of the state’s Food Code.

    In 2025, the majority of Triangle food establishments passed with A and B letter grades, which are at least 80%. But some scored lower than that.

    Of the thousands of restaurants inspected, here’s which ones in the area received C grades this year and grades they received upon reinspection.

    Worst Triangle sanitation scores

    Urban Turban Indian Grill and Bar at 1125 W. N.C. 54, Suite 301 in Durham

    • The restaurant received a score of 74.5% on Wednesday, Nov. 19 and was in violation of 21 standards.
    • Violations included food splatter on walls in multiple areas, cleaning needed on the floors throughout the entire kitchen and multiple foods without dates.
    • The restaurant previously scored 88% in August 2025 and 96.5% in May 2025.
    • Follow-up: It received a 88.5% after it was reinspected Tuesday, Dec. 23.

    Dimsum Asian Bistro at 5410 N.C. 55 Y in Durham

    • The restaurant received a score of 77% during an inspection on Friday, Feb. 28 and was in violation of 21 standards.
    • Violations included multiple knives stored as clean with visible debris on blade, some gnats throughout the kitchen and raw meat stored above dumplings in flip top single door and raw meat stored above ice-cream out of original packing in two-door freezer.
    • The restaurant previously scored 90% in June 2024 and January 2024.
    • Follow-up: It received a 97.5% when it was reinspected Friday, March 28 and a 92% on Tuesday, Sept. 19.

    Ni Armor Hawaiian BBQ at 517 West St. in Pittsboro

    • The restaurant received a score of 78% during an inspection on Wednesday, Sept. 3 and was in violation of 19 standards.
    • Violations included multiple dirty food pans, utensils, utensil bins and fly strips touching clean food pans, mouse droppings observed inside of clean food bins and numerous flies observed near a three-comp sink.
    • The restaurant previously scored 93% in January 2025 and April 2025.
    • Follow-up: It received a 96% when it was reinspected Thursday, Oct. 9.

    There were not any food establishments in Wake, Orange or Johnston counties that received C grades in 2025.

  • Roaches found during Fort Worth health inspections, 1 restaurant closed

    One Fort Worth restaurant was closed from Nov. 30 to Dec. 13 after health inspectors found water issues, and roaches were found at several facilities, according to data from the city compiled by the Star-Telegram.

    King Wok at 4544 Golden Triangle Blvd. scored 27 on Dec. 5. The inspector noted a buildup of residue on the drink machine nozzles, uncovered tubs of raw meat in the freezer and roaches on the ceiling and wall near the three-compartment sink.

    There was no hot water at any of the sinks, and the city required King Wok to close until the problem with the water could be fixed.

    Fort Worth uses a demerit system, with zero being a perfect score. Restaurants that receive more than 30 demerits are required to fix the worst problems immediately and clean up the rest of the violations within 48 hours. No restaurants scored more than 30 during this round of inspections, but several received high demerit scores.

    Sunny’s Cafe at 3000 SE Loop 820 scored 26 on Dec. 9. Lettuce and other produce were stored at an unsafe temperature, a certified food manager wasn’t at the restaurant during the inspection and some employees weren’t able to prove they’d completed an accredited food handler course.

    Habaneros Fresh Mex Cafe at 5601 Basswood Blvd. scored 25 on Dec. 4. The inspector found a dirty onion chopper, a chemical spray bottle near the dry foods area and no paper towels or soap at one of the hand sinks.

    Habaneros received a score of 2 during the Dec. 11 follow-up inspection.

    Prima’s Pasta and Pizza at 6108 S. Hulen St. scored 13 on Dec. 2. The inspector noticed one dead roach, personal items on some of the counters and bread stored on the floor.

    Sbarro Pizza at 4800 S. Hulen St. scored 11 on Dec. 4. Some macaroni and cheese and lasagna were held at an unsafe temperature and dead roaches were found in the light fixture.

    El Merendero Restaurant at 3000 South Fwy. scored 10 on Dec. 5. The inspector found a dying roach near the mop sink and the outside dumpster lid was left open.

    Press Cafe at 4801 Edwards Ranch Road scored 6 on Dec. 12. One live roach was found in the facility and there was an accumulation of food debris on the stove.

    Eleven establishments, including the Park Vista Children’s Academy daycare at 13033 Park Vista Blvd., received a perfect score of zero.
