    ‘Misfire and explosion’ leave pyrotechnic workers injured at La Puente fireworks show

    A pyrotechnics worker was seriously injured and two other fireworks professionals suffered minor injuries Wednesday night due to a mishap during a show sponsored by the city of La Puente.

    “Our hearts are heavy as we share news about the La Puente’s Fortunate Jimenez Fireworks Show,” the city posted in a statement on social media. “One dedicated pyrotechnic technician was seriously injured due to a misfire and explosion. Two other technicians suffered minor injuries.

    “We ask for your thoughts and support for their families during this challenging time.”

    Those injured “were giving the community a wonderful 4th of July display,” the statement added.

    The incident apparently brought the show to a halt. “We thank the community for your patience and understanding,” officials stated.

    The city had promoted the event online as a “family-friendly beer garden celebration” that included live music and food, with festivities beginning at 4 p.m. at the La Puente Baseball Field.

    One witness, Tiffany Angulo, said on Facebook: “The firework did blow too low. Twice! I literally said, ‘God forbid anyone get hurt.’ Whatever company we purchased the fireworks from I hope we don’t again.”

    “We saw the explosion and asked staff if anyone was injured,” posted Maggie Perez Martinez in response to La Puente’s announcement. “They said, ‘No. everyone’s fine.’ So sorry to hear this.”

    Vincent A. Barela added: “So sad to read this, we were there and the show ended abruptly. Hope the show runners recoup quickly and with minimal injuries.”

    A Wednesday night report from NBC4 showed a small fire burning on the ground amid the pyrotechnic equipment. From the news helicopter, it looked as though fire crews and police officers were remaining clear temporarily of the potentially dangerous area.

    One social media poster, Shirley Garay-Bermudez, saw the incident as a sad, teachable moment: “This explains why fireworks are dangerous… The pyrotechnics are trained and do wear protective gear and they are aware anything can go wrong, which is why the fire department and paramedics are on site in cases of emergency. Prayers for all who were injured.”

    HOOD RIVER, Ore. — It’s a question she asks a lot. “How many of you here have an emergency plan?” Nora Yatsov is a professional emergency manager. She works for PacifiCorps.

    “An emergency plan doesn’t take any money. It just takes a little bit of time, some conversations.  Find out what you and your family, those that rely on you, what to expect during a disaster, where are you going to meet if you can’t get home.”

    She’s telling people at a community gathering in Hood River what they should do to have a plan in place now before the weather turns hotter and the danger of wildfires increases.

    “What are your important contacts’ information, do you know all the phone numbers in your phone? Do you have them memorized? I don’t. Write them down, make sure they’re accessible.  Important financial information or medications or prescription information.  It just takes a little bit of time to wrap all that together.   And put it in a document or in a folder that you can have with you.  You never know when you could be in a situation where you don’t have access to your resources.”

    When she speaks to groups, Yatsov also asks, “How about a kit? Who has a kit here?”

    That kit is vital, she says.  “We really encourage everybody to have a kit, not just power outages. We live in Cascadia subduction zone territory here, right? We have a lot of other hazards that we deal with.”

    She lists for what needs to be in that kit, now, before the hotter weather boosts our risk of wildfires.   “For a home kit, we encourage a two to three week supply of food, shelf stable food, and food that you like to eat, not MREs, because who wants to eat that when you’re stuck in a disaster?  A first aid kit, of course.  If you have any pets, a leash and food for them, or a kennel, or some way to contain them, because they get scared. For kids, comfort items, because they’ll be scared. Things that make them feel safe and comfortable.  Activities to keep them distracted.   For those that need some assistance walking devices or anything else like that. Make sure you have those assistive devices available for them.”

    A United Airlines plane external panel discovered missing after Oregon flight

    A section of a United Airlines aircraft was discovered missing upon inspection Friday afternoon in Southern Oregon, adding to a growing list of mishaps for the airline.

    United Airlines Flight 433 took off from San Francisco and successfully landed 90 minutes later at Rogue Valley International Airport in Medford, Ore., at 11:53 a.m.

    Airport personnel noticed a “piece from the underside of the plane,” a Boeing 737-800, was missing upon a routine postflight inspection, Airport Director Amber Judd told The Times.

    “Our airport operations were paused briefly so that we could conduct a runway safety check to look for debris,” Judd said. “We did not find anything.”

    Judd said the plane landed safely and all 139 passengers and six crew members exited without an issue.

    The flight was scheduled to continue to Denver, but was initially delayed 3 hours and 35 minutes before eventually being canceled.

    “It’s my understanding that most passengers were aware of the delay and the circumstances, although there were probably some that didn’t know,” Judd said.

    United Airlines in a statement Friday said the aircraft’s crew did not declare an emergency to airport personnel as “there was no indication of the damage during flight.”

    “After the aircraft was parked at the gate, it was discovered to be missing a panel,” United’s statement read. “We’ll conduct a thorough examination of the plane and perform all needed repairs before it returns to service.”

    The airlines also said it would conduct an investigation.

    Judd said the plane was an older 737-8 and not one of the Boeing Max aircrafts that have received scrutiny in January after a door panel blew off an Alaska Airlines flight that left Portland, Ore.

    Nonetheless, four Boeing planes operated by United have suffered incidents over the last two weeks.

    A Boeing spokesperson referred all questions to United Airlines regarding the airline’s fleet and operation.

    On Monday, a San Francisco-bound United Airlines flight turned around two hours after leaving Sydney. The Boeing 777-300 aircraft returned due to a maintenance issue.

    Prior to that, a Boeing 777-200 operated by United Airlines made an emergency landing in Los Angeles after a tire fell off on March 7.

    There was also an emergency landing in Houston on March 4 after flames were spotted coming from a United Airlines Boeing 737-900ER. United confirmed the engine ingested bubble wrap.

    Four days later, a Boeing 737-8 Max rolled onto the grass near a runway in Houston upon landing, though no passengers were injured.

    United stressed their were no injuries in any of these incidents.

    “We take every safety event seriously and will investigate each of the incidents that occurred this month to understand what happened and learn from them,” the United statement said. “Much of this work is conducted together with the manufacturers, the FAA, and the NTSB as well as with the manufacturers of individual components.”

    New ambulance service for Montgomery County

    MONTGOMERY COUNTY, N.Y. (NEWS10) — One Capital Region County is getting a new ambulance service after cutbacks forced law enforcement to pick up the slack. NEWS10 has the latest on the service that just kicked off on Monday.

    “Really it’s all about patient care and getting an ambulance to somebody who needs it in a timely manner,” said Sergeant Justin Smith with the Montgomery County Sheriff’s Office.

    Smith will oversee the sheriff’s office EMS division. He tells NEWS10 that it took nearly three years of planning to get to this point.

    “It’s been a long process so it’s kind of a Bittersweet moment here today we’ll see where this takes us. We built this from the ground up and we want to build it the right way,” said Smith.

    The county saw cutbacks in the industry after volunteer ambulances began to shutter. They relied on just a few companies to maintain EMS service.

    Within a couple of hours, the ambulance is already in use. Our cameras were rolling when the ambulance went on its first call, a non-emergency transport of a patient to St. Mary’s Hospital.

    Amsterdam Fire Chief Anthony Agresta says Monday was the perfect example of why this ambulance is needed. “We were in a situation where one of our two ambulances is out of service,” said Agresta.

    The county hired 16 people to staff the ambulance, two of them full-time. Two of them spoke to NEWS10 on their first day on the job. They tell me they live in the communities they serve and have seen the need for more ambulances.

    “It’s definitely a nice way to impact the community I live in,” said EMT Anne Clouthier.

    “It will be great to have improved EMS coverage in the county,” said EMT Michael Kent.

    Montgomery County Executive Matthew Ossenfort praises the Sheriff’s Office for the new service. “Today the real exciting part is we’re getting off the ground, we got it started and we’re here to supplement and work with providers who are already here,” said Ossenfort.

    An ill-advised ocean outing turned into a fight for survival Sunday afternoon for 19 people after a boat crashed into the rocky breakwater off Long Beach as a powerful storm lashed the Southland.

    “Apparently they’d gone out sailing and met with some gale-force winds,” said Brian Fisk, a firefighter and public information officer for the Long Beach Fire Department.

    The 40-foot sailboat found itself in choppy, storm-riled waters when winds snapped its mast, rendering it difficult if not impossible to control. The craft ended up battered and tossed against the Long Beach breakwater near the mouth of Alamitos Bay, with those aboard scrambling up the rocks, temporarily safe but stranded and in danger.

    The distress call came in to the Long Beach Fire Department at 2:50 p.m. over Channel 16, which is reserved for emergencies, Fisk said. The department sent two rescue boats and lifeguards.

    Before the professional rescuers arrived, eight people already had been extricated — either by people who heard about the situation on a scanner, were alerted by those on board or saw what was happening. Fire Department rescuers brought the remaining 11 to safety.

    “The weather not only caused the accident,” Fisk said, “but hampered our rescue effort.”

    One person suffered non-life-threatening injuries.

    “Those people were really lucky,” Fisk said.

    'Thousand-year storm' leaves San Diego reeling from punishing rainfall, floods

    In a matter of minutes Monday morning, communities across southeastern San Diego were transformed into disaster zones: Families fled their homes in chest-deep floodwaters; vehicles were swept downstream as roads became rivers; residents cried for help from their rooftops.

    A deluge of rainfall from what city officials are calling a “thousand-year storm” forced hundreds of rescues, flooded an untold number of homes and businesses and caused millions of dollars in estimated damage. The floodwaters had mostly receded by Tuesday afternoon, revealing the devastating aftermath of California’s latest climate emergency — and leaving hundreds without housing and transportation, and with ruined valuables and personal belongings.

    “The damage and the impact was absolutely devastating,” San Diego Mayor Todd Gloria said at a Tuesday news conference. “Entire lives changed in just a few minutes.”

    “The amount of water that we saw yesterday would have overwhelmed any city drainage system,” he said. “This dumping of rainwater is unprecedented in most San Diegans’ lifetimes. None of us alive have seen anything quite like this.”

    More than 4 inches of rain fell in several areas in and around San Diego on Monday — much of it in just a few hours — a historic rainfall event, according to Elizabeth Adams, a meteorologist with the National Weather Service in San Diego. The airport recorded 2.73 inches, more than its typical total for the entire month of January.

    “That is not only the wettest January day on record, but it’s the fourth-wettest day of any calendar day” for San Diego, Adams said. Many areas saw rainfall rates well above three-quarters of an inch per hour. Over half an inch per hour can easily cause dangerous flash flooding.

    “It’s a ton,” Adams said. “Pretty much anywhere in the country that receives 3 to 4 inches in a three- to four-hour time period is going to see flooding.”

    Parts of San Diego were completely inundated.

    The city’s southeastern neighborhoods, including Southcrest, Mountain View, Encanto, Logan Heights and San Ysidro, saw some of the worst damage.

    Gloria said city and county leaders are focused on recovery. Both the city and county declared a local emergency. The mayor estimated, conservatively, that the storm caused $6 million in damage, but officials say assessments are far from complete.

    Gov. Gavin Newsom on Tuesday proclaimed a state of emergency for San Diego and Ventura counties, both of which have been walloped by wet winter storms. At the end of December, torrential downpours in and around Oxnard caused similar damage. During that event, Oxnard saw rainfall rates of 3 inches an hour, one of the heaviest downpours ever recorded in the area.

    The worry now is that the number of people displaced in San Diego could continue to grow in the coming days. Though no official figure was provided Tuesday, city leaders said they estimated hundreds had been forced from their homes, at least temporarily.

    “What was generally assumed to be the impact yesterday … was probably an underestimate,” said San Diego City Council President Sean Elo-Rivera, whose district includes some of the communities that saw the worst of the flooding. He said he visited many of those residents early Tuesday, touring a whole apartment complex that took on water, likely displacing dozens of families.

    The American Red Cross is operating two emergency shelters at Lincoln High School and Bostonia Recreation Center. As of Tuesday, the nonprofit said 18 households — more than 50 people — had registered to stay. But with so many people probably still returning home after fleeing, Elo-Rivera said he expected those numbers to rise. City and county officials are asking residents to fill out a voluntary survey about flood damage.

    “I think it’s going to take a little bit more time to realize the extent of the damage,” Elo-Rivera said.

    On Monday afternoon, Manuel Deleon was unexpectedly called back to the office during his shift driving a tow truck — only to find the office flooded. Roaring water had swept away his personal vehicle.

    “The water was out of control,” said Deleon, 47. “My car slipped with the mud and went right into [a nearby] ditch and it was just fully submerged.”

    Deleon, whose 2007 BMW was one of dozens of cars carried away in the flash floods, said he wasn’t sure how he’d get to work in the coming days. He attempted to clean the soggy and caked-in mud from the interior, but that was a lost cause.

    “This rain took everybody by surprise,” he said. “It’s crazy.”

    San Diego Fire Chief Colin Stowell said his crews made at least 150 rescues Monday, in addition to 30 animal rescues.

    “We literally saw over 100 rescues in the Southcrest neighborhood alone,” Stowell said.

    “Luckily we saw very few injuries and no fatalities,“ Stowell said, calling that feat “remarkable” given the extent of the emergency.

    More than 1,000 people remained without power Tuesday, after widespread outages Monday, according to the San Diego Gas & Electric outage map.

    Although much of San Diego was under a flood watch all day Monday, city officials said they were not prepared for the extent — and speed — of what came down.

    “Nobody anticipated the severity of the storm,” Gloria said. “We got a lot more rain than [what was predicted] in a much shorter amount of time.”

    He said he planned to meet with the National Weather Service to discuss the disparity between forecasts and what occurred but emphasized that his teams were currently focused on recovery.

    Adams said the circumstances Monday ended up being a perfect storm for rare, heavy rainfall in San Diego: extreme atmospheric moisture and a storm path over its downtown — which forecasters warned residents about as soon as possible, she said.

    Just after 8 a.m. Monday, the agency issued a flash flood warning for a stretch of coastal communities just south of Orange County, including Oceanside, Carlsbad and Vista. Soon after, a larger stretch of southwestern California was placed under a flash flood warning.

    “We used pretty intense warnings,” Adams said. “We tried to really heighten the message … [that] this is a really dangerous situation that doesn’t happen in San Diego proper that often.”

    The day before the storm, the National Weather Service’s forecast discussion warned that the ground, already saturated from storms over the weekend, could heighten flood concerns. But forecasters said it was still hard to predict how much rain would fall, and where.

    By Monday morning, Adams said the situation developed rapidly, with that intense atmospheric moisture — what she called 250% to 350% of normal — and the direct storm path aligning.

    That “really lead to torrential rainfall across the county, but especially focused on downtown and the surrounding neighborhoods,” Adams said.

    City officials said these extreme circumstances are likely to become a new normal requiring more preparation, coordination and investment.

    “This is called climate change. It is real, it is happening,” Gloria said, “and we experienced it yesterday in San Diego.”

    Officials agreed that the city’s outdated stormwater drainage system, for which $2 billion of necessary work hasn’t been budgeted, didn’t help.

    Elo-Rivera said he would like to see those much-needed funds allocated, and in an equitable way — noting that many of the communities affected most were working-class, with a majority of Latino and Black residents.

    These communities “have long been under-invested in and divested in and ignored by the city,” he said. “Public investment in climate resiliency is incredibly important … [especially] prioritizing the communities that have been left behind and are most likely to be devastated by events like yesterday.”

    California doctors analyzed medical emergencies at immigration detention facilities. Here’s what they found

    An investigation published Wednesday in the Journal of the American Medical Assn. found discrepancies between emergencies at California immigrant detention facilities that were reported to local authorities and those reported publicly by the federal government.

    The study, led by Dr. Annette Dekker, an assistant professor of emergency medicine at UCLA, analyzed about 1,200 emergencies from 2018 through 2022 at three detention centers: the Adelanto ICE Processing Center in San Bernardino County, the Otay Mesa Detention Center in San Diego and the Imperial Regional Detention Facility east of San Diego. Most of the patients involved were men, and the median age was 39. Local authorities responsible for emergencies at two other facilities in California did not provide data.

    Private prison companies manage seven immigrant detention facilities in California on behalf of U.S. Immigration and Customs Enforcement, with space to detain more than 7,000 people.

    Psychiatric emergencies made up just 48 of the EMS responses reviewed by Dekker and four other California doctors — less than 4% — despite significantly higher rates of such complaints reported by ICE during the same period.

    At each facility, the number of psychiatric emergencies reported to EMS was less than or equal to the number of suicide attempts reported by ICE. When other ICE-reported mental health issues that required observation were added to that total, Dekker said, there was a tenfold increase over the total reported by local EMS agencies.

    Dekker said she believes that discrepancy could be interpreted to mean that all but the most extreme psychiatric crises are being appropriately treated by in-house medical staff. A 2021 California Department of Justice inspection review highlighted significant mental healthcare deficiencies at all three facilities, including understaffing and care delays.

    “It’s possible someone has a suicide attempt that doesn’t require EMS, but speaking as an emergency physician, there are a lot of different psychiatric emergencies — some related to [suicidal] ideation, some related to psychosis — so it is odd to me that the number of EMS-reported psychiatric emergencies isn’t higher,” Dekker said. “Why aren’t the medical staff reaching out for more help?”

    Medical reviews aren’t required unless a detainee dies in ICE custody, the report notes.

    “Systematic substandard care has been identified as a factor associated with these deaths, including a lack of recognition of severe illness, medical staff dismissal of concerns about individuals’ health, and delays in activating external emergency care,” the report states. “These findings suggest that there are near misses not captured in death reviews.”

    ICE did not immediately respond to a request for comment.

    Detainees cannot access 911 and must rely on staff to call on their behalf. Broadly, death reviews show that some responses to requests for help have been delayed.

    At the Adelanto and Imperial facilities, the rates of EMS-reported emergencies were lower for women than men. But at Otay Mesa, the rate was significantly higher for women, including 12% of emergencies related to pregnancy concerns.

    An ICE directive issued on July 1, 2021, says “ICE should not detain” people who are pregnant, postpartum or nursing and should ensure their expeditious release from custody. Seven EMS-reported emergencies for pregnancy-related concerns were recorded after the directive took effect, “indicating that Otay Mesa continued to house pregnant individuals despite ICE directives,” the report noted.

    Authors of the JAMA report said it is unclear whether the higher rate of emergencies among female patients at Otay Mesa represents higher rates of illness or better detainee health monitoring. But the 2021 California DOJ inspection noted that all three facilities “impermissibly house female detainees in restrictive housing under conditions disparate to those of male detainees.”

    More than a quarter of patients at all three facilities had at least one abnormal vital sign reading during their encounter with medics — most frequently an elevated heart rate. Among all three facilities, the top three primary symptoms reported by patients were chest pain, abdominal pain and altered mental status. Traumatic injury was also among the issues most frequently reported by EMS providers.

    The rate of emergencies at all three facilities increased amid the pandemic despite efforts to reduce capacity at the facilities during that time.

    Between January 2019 and December 2021, there were 742 EMS-reported emergencies. During the same period, ICE reported 1,481 medical emergencies. Dekker said the discrepancy suggests that EMS data fail to capture the total number of medical emergencies occurring in ICE facilities.

    Unlike ICE, publicly funded healthcare systems such as Medicare and Medicaid have greater government oversight that requires them to report more rigorous metrics, Dekker said. She said recent ICE facility reports contain less data than in years past and no longer include metrics such as off-site medical emergencies and the number of suicide attempts.

    “I have walked away with significantly more questions than when I started,” Dekker said of the report. “The bottom line is that we need expanded reporting from ICE on health outcomes to understand what’s going on.”

    Emergency services are at a crash in central Rotorua.

    Emergency services are responding to a crash between a mobility scooter and another vehicle in central Rotorua.

    A spokesman said police were called to a two-vehicle crash about 10.20am.

    The crash is near the Tutanekai St and Amohau St intersection and the road is closed.

    A Fire and Emergency NZ spokeswoman said it responded with three trucks about 10.20am.

    She said the crash involved a mobility scooter and a car according to information recorded in the Fenz system.

    A reporter at the scene said a mobility scooter could be seen, along with a motorcycle on the side of the road being photographed by police.

    The Serious Crash Unit has been advised, police said in a statement.

    Amohau St is blocked between Fenton St and Amohia St, and diversions are in place.

    The report claims that Aditya Dhar and the team have researched the subject for months. They also have detailed information on the assassinations. “Much like URI, this one too will be a thrilling and exciting story to unfold on the big screen. He plans to mount it on a big screen and will take on floors in April – May 2024,” the report said. The one untitled thriller which will soon proceed to the casting stage. He wants an A-list actor to spear the mission-based movie. Aditya also has three productions for release. It includes Dhoom Dhaam starring Yami Gautam and Pratik Gandhi, a political thriller and Baramulla.

    Well, Aditya Dhar’s movie is not just the one which will be based on the India Vs Pakistan war. There is Lahore, 1947, starring Sunny Deol which will be produced by Aamir Khan. There’s also Sam Bahadur starring Vicky Kaushal. The movie is a biopic on the life of Sam Manekshaw. Salman Khan and Katrina Kaif starrer new movie Tiger 3 also has an India-Pakistan angle to it. There is also Sky Force. The Akshay Kumar, Veer Pahariya starrer movie is set around India Pakistan Air war. Emergency starring Kangana Ranaut is also in the pipeline. There’s talk about Border 2 as well. And then, Gadar 2 makers hinted at Gadar 3 too.

    Dangerous winter weather with temperature lows in the teens is predicted to hit Central Texas this week. With some of our animal enclosures exposed to the elements, temperatures this low severely strain our facility so we must get our Town Lake Animal Center shelter pets in warm homes by THIS THURSDAY. We’re calling on our community members for help needed NOW:

    1. Foster homes for cold shelter animals
      We need our animals, especially our dogs, out now, before the coldest weather hits Thursday. With Christmas week/holiday travel, we anticipate it will be very difficult to find fosters so if you are staying in the Austin area your help is urgently needed. Email [email protected] or come to our Town Lake Animal Center location (1156 W Cesar Chavez St.) between noon-6pm to foster. We do ask that you keep them in your home until at least Monday, December 26th.

    2. Supplies or monetary donations
      With facility and extra resource needs to handle this emergency, we are seeking monetary donations now. Click here to make a gift to help fund our response and operations. We also need people to donate any of the following supplies to be two-day shipped or bring them this week to our Town Lake location. (Drop off in front of building C.) Some of these will be sent to the neighboring shelters we support as well.
      • Blankets (ideally small, fleece – NO sleeping bags)

      • Medium & large dog coats

      • Moving blankets

      • Tarps

      • Waterproof/self-warming dog blankets

      • Self-warming heating pads

      • Space heaters

      • Heat lamps

    3. Other help for shelters we support
      Many neighboring cities’ animals are also at risk during weather emergencies like this, and are often far less resourced than Austin. We are also asking for help getting the following urgent needs met for these shelters this week (list will update as we receive requests for help):
      • The City of Devine Animal Shelter needs an adopter or foster for the animals here, especially puppies.

      • San Benito needs and adopter or foster for a cat and their dogs here

    4. Education and help for community pets
      Read and share this checklist to protect pets where you are.

    • Bring pets inside. The best thing you can do for your pet is to bring them inside with you. While some breeds of dogs are more tolerant of cold weather than others, no pet should be left outside for long periods of time when it is below freezing (32ºF). You know your pet best, so be vigilant about watching for signs of their cold tolerance and limit outdoor activities accordingly.

    • Check your car for cats. Our feline friends like to hide from this weather in car engines and/or wheel wells, so thump the hood of your car a few times and check your wheels for stowaways before you start the engine and take off.

    • Provide a makeshift enclosure for outdoor animals. If you’ve noticed outdoor cats or other animals in your community suffering from the cold (shaking, curled up, etc.) and you are worried about them, create a makeshift shelter for them to stay warm in. A closed box or Rubbermaid bin with a cut out in the side, with towels or blankets, will help keep them safe in the frigid temperatures.     • Or consider opening your garage slightly (and leaving a heating pad or heat lamp on) to let cats in from the cold.

    • Put a sweater on your pup. If you have a dog with a short coat, you can keep them a bit more insulated by putting a sweater or dog coat on them. Be sure the sweater and coat are completely dry for each outing, though, as damp or wet outerwear could actually make them chillier.

    • Check paws. After outdoor activity, check your pet’s paws for any signs of cracking on the paw pads, redness between toes, or bleeding. Wipe them down after each outing, too, to remove any salt, ice, or chemicals.
