A Bronx man found shot in the head in his apartment two weeks ago has died, according to police who apprehended the shooter after he fled to Pennsylvania.

Cops responding to a report of an assault inside a first-floor apartment on East Mosholu Parkway South near E. 204th St. in Jerome Park on Aug. 20 found Phillip Martinez, 36, sprawled out on the ground, shot in the head, cops said.

EMS rushed him to St. Barnabas Hospital, where he died of his injuries three days later.

Investigators quickly identified Jason Soto, 44, as the man who shot Martinez, but Soto had already fled the city, cops said.

A day after the shooting, Soto was apprehended in Ben Salem, P.A., some 90 miles outside the Bronx, after being pulled over for a traffic violation, cops said.

Knowing he had fled the city, cops put out an alert about Soto to neighboring states, a police source with knowledge of the case said.

A man who had been shot in the head on Aug. 20 in an apartment building along East Mosholu Parkway South in the Bronx has died, officials said.

Cops in Ben Salem held Soto until Thursday when NYPD Bronx Warrants detectives extradited him back to New York and charged him with murder and manslaughter.

His arraignment in Bronx Criminal Court was pending Friday.

Neither man lived in the apartment Martinez was shot in, police said.

Martinez lived on the other side of the Mosholu Parkway, about four blocks from where he was shot, cops said. Soto lives in Camden, N.J. and has no arrest history in New York City, cops said.

It was not immediately disclosed if Soto and Martinez knew each other or what the pair were arguing over.
