What is Chronic Venous Insufficiency and Why Your Legs Are Crying for Help – Southwest Journal

Leg swelling after a long day is easy to dismiss. The same is true of aching calves, ankle discoloration or varicose veins that have slowly become more visible.

When those problems return day after day, they may point to chronic venous insufficiency, or CVI. The condition develops when veins in the legs cannot move blood back toward the heart efficiently. Blood then collects in the lower legs, raising pressure inside the veins and surrounding tissue.

Early symptoms can feel minor. Legs may become heavy by late afternoon or shoes may feel tighter around the ankles. More advanced disease can damage the skin and produce wounds that remain open for months.

CVI does not improve simply because a person becomes used to the discomfort. A medical evaluation matters when swelling, pain or skin changes persist, especially for anyone with a history of blood clots.

What Is Chronic Venous Insufficiency?

Chronic venous insufficiency is a long-term circulation problem involving the veins of the legs.

Healthy leg veins contain small valves that open as blood moves toward the heart and close to stop it from flowing backward. Movement of the calf muscles also helps push blood upward whenever a person walks.

When the valves weaken or become damaged, part of the blood falls back toward the feet. Pressure builds inside the veins, leading to swelling, inflammation and gradual changes in the skin.

The condition belongs to a wider group of problems called chronic venous disease. A person may begin with visible spider veins or varicose veins and later develop ankle swelling, skin discoloration or an open ulcer.

Estimates of how many Americans are affected differ because studies use different definitions. Varicose veins and milder forms of venous disease are widespread, but advanced CVI is less common than claims that 40% of all Americans have the full condition.

Women are diagnosed more frequently, partly because pregnancy and hormonal changes place additional strain on the veins. Risk also increases with age, previous deep vein thrombosis, obesity and long periods spent sitting or standing.

Common Symptoms of Chronic Venous Insufficiency

Symptoms tend to develop gradually and often become more noticeable later in the day.

Aching, heaviness and swelling may worsen after several hours of standing or sitting. Elevating the legs or walking can bring relief because both actions help blood leave the lower legs.

Common signs include:

  • Swelling around the ankles and lower legs
  • A dull ache, pressure or feeling of heaviness
  • Leg cramps, throbbing or burning
  • Visible varicose veins
  • Itching, flaking or irritation near the ankles
  • Brown, red or purple skin discoloration
  • Skin that becomes firm, thick or tender
  • Open wounds near the ankle

Small veins around the ankle can become unusually visible before more serious skin changes appear. Doctors sometimes call this pattern corona phlebectatica. It can be an early clue that pressure inside the leg veins has remained high for a long period.

Some people also report restless sensations, tingling or night cramps. Those symptoms are not specific to CVI and can occur with nerve disorders, medication side effects and other circulation problems.

Venous ulcers usually appear near the inside of the ankle. They are often shallow, moist and surrounded by discolored or hardened skin. The University of Rochester Medical Center notes that these wounds can be difficult to heal without treatment of the underlying circulation problem.

Who Is Most Likely to Develop CVI?

No single factor determines who will develop chronic venous insufficiency. Risk rises when several factors are present together.

A previous deep vein thrombosis is among the most important. A clot can block blood flow and permanently damage valves inside a deep vein. The resulting problem is known as post-thrombotic syndrome and may appear months or years after the clot.

Other important risk factors include:

  • A family history of varicose veins or chronic venous disease
  • Older age
  • Pregnancy, particularly multiple pregnancies
  • Obesity
  • Limited physical activity
  • Smoking
  • Previous leg injury or surgery
  • Jobs that require prolonged sitting or standing

Pregnancy combines several sources of pressure on the veins. Blood volume increases, hormones relax vein walls and the growing uterus can restrict blood returning from the legs. Symptoms improve after delivery for many women, but repeated pregnancies can leave lasting valve damage.

People working at desks, checkout counters, hospitals, factories and similar settings also face higher risk when they remain in one position for hours. The problem is not the occupation itself. Lack of regular calf movement reduces the pumping action that normally helps return blood to the heart.

Height has also been associated with venous disease, since blood must travel a greater distance against gravity. It is a smaller factor than blood-clot history, obesity or prolonged inactivity.

What Causes Chronic Venous Insufficiency?

CVI develops when pressure inside the leg veins remains elevated over time.

Damaged valves are one reason. A valve that does not close properly allows blood to flow in both directions instead of moving steadily toward the heart. Veins can then widen, making the valve problem worse.

A blocked vein can produce the same result. Deep vein thrombosis may narrow or obstruct the route that blood normally follows. The body can create alternative channels, but they may not carry blood efficiently enough to prevent swelling and pressure.

Calf weakness and inactivity also matter. Each step contracts the calf muscles around the deeper veins, helping move blood upward. Long periods without movement remove that assistance.

The main causes and contributing factors include:

  • Weak or damaged vein valves
  • Previous deep vein thrombosis
  • Obstruction or narrowing of a major vein
  • Reduced calf-muscle function
  • Pressure associated with obesity or pregnancy
  • Congenital abnormalities of the veins

Varicose veins and CVI are connected but are not identical. A person can have visible varicose veins without advanced insufficiency. Another person can have deep-vein damage and significant swelling without prominent surface veins.

CVI Can Become More Serious Over Time

Persistent venous pressure affects more than the veins themselves. Fluid and inflammatory cells move into nearby tissue, causing swelling and gradual skin damage.

One early complication is stasis dermatitis. The skin becomes itchy, red, dry or scaly and can resemble eczema. Scratching may create small breaks that allow bacteria to enter.

The skin can later become thick and hardened, a change known as lipodermatosclerosis. The lower leg may narrow above the ankle as tissue becomes scarred.

Other complications include:

  • Recurring skin infections
  • Bleeding from fragile varicose veins
  • Chronic pain and reduced mobility
  • Venous ulcers
  • Repeated ulcer infections
  • Reduced ability to work or remain active

Venous ulcers create the greatest long-term burden. They can drain fluid, limit walking and repeatedly close and reopen. Healing requires wound care along with treatment of the pressure that caused the wound.

Bleeding from a varicose vein can also look alarming. Elevating the leg and applying firm direct pressure usually slows the bleeding, but emergency care is necessary when it does not stop.

How Doctors Diagnose Chronic Venous Insufficiency?

Diagnosis begins with a medical history and physical examination.

A clinician will ask when swelling begins, what makes it better or worse and whether the patient has had a blood clot, leg injury or previous vein treatment. Both legs are examined for swelling, varicose veins, discoloration, hardened tissue and ulcers.

Duplex ultrasound is the main test used to confirm CVI. It combines standard ultrasound imaging with Doppler measurements that show the direction and speed of blood flow.

The examination can reveal:

  • Valves that allow blood to flow backward
  • Old or current blood clots
  • Narrowed or blocked veins
  • The veins responsible for abnormal pressure

Ultrasound is painless and does not use radiation. Testing may be performed with the patient standing because reflux can be easier to detect when gravity places pressure on the veins.

More advanced imaging is reserved for complicated cases. A venogram, CT venography or magnetic resonance venography may be ordered when doctors suspect a blockage in the pelvis or abdomen.

Clinicians also need to rule out other causes of leg swelling. Heart failure, kidney disease, liver disease, lymphedema, infection and certain medications can produce similar symptoms.

Peripheral artery disease deserves special attention before compression treatment begins. Strong compression may be unsafe when blood flow through the arteries is severely reduced. An ankle-brachial index or another arterial test may be needed first.

How Doctors Measure the Severity of CVI?

Specialists use the CEAP system to describe chronic venous disease. The letters refer to clinical findings, cause, anatomy and the way the disease affects blood flow.

The system helps doctors describe how far the disease has progressed and compare treatment results. It does not replace an individual treatment plan.

Treatment Options for Chronic Venous Insufficiency

Treatment depends on the cause, the severity of symptoms and the condition of both the veins and arteries.

Early care focuses on reducing swelling and helping blood move out of the lower legs. Procedures become relevant when symptoms remain troublesome, ulcers develop or ultrasound identifies a vein that can be treated directly.

Compression Therapy

Compression stockings apply graduated pressure, with the strongest squeeze near the ankle and less pressure higher on the leg. The pressure limits swelling and supports blood flow toward the heart.

Compression strength and length should be chosen with a clinician. Knee-high stockings work for many people, but thigh-high garments, wraps or adjustable devices are used in other cases.

A stronger stocking is not automatically better. People with peripheral artery disease, severe heart failure, fragile skin or certain nerve conditions need medical guidance before using compression.

Stockings work only when they fit properly and are worn consistently. They are generally put on in the morning before swelling develops and removed before bed.

Walking and Leg Movement

Walking activates the calf-muscle pump and is one of the most useful daily measures for CVI.

People who sit for work should stand and move regularly. Those who stand for long periods can alternate positions, perform calf raises or take brief walking breaks.

Exercise does not repair a damaged valve, but it can improve circulation, mobility and weight control.

Leg Elevation

Elevating the legs above heart level allows gravity to help drain pooled blood and fluid.

Short sessions several times a day can reduce swelling. Resting the feet on a low stool while remaining upright provides less benefit than lying down with the lower legs properly supported.

Skin and Wound Care

Dry or inflamed skin should be cleaned gently and moisturized to reduce cracking. Prescription creams may be used for stasis dermatitis.

Venous ulcers require structured wound care. Treatment may include compression bandaging, absorbent dressings, removal of dead tissue and infection management.

Antibiotics are not routinely needed for every ulcer. They are used when there are signs of infection rather than simply because a wound is open.

Sclerotherapy

Sclerotherapy involves injecting medication into an abnormal vein. The treatment irritates the vein lining, causing the vein to close and gradually disappear.

Blood is redirected through healthier veins. The method is used for some varicose veins and smaller abnormal branches.

Endovenous Ablation

Radiofrequency and laser ablation close an unhealthy vein from inside using heat delivered through a thin catheter.

The procedure is performed through a small puncture, usually with local anesthesia. Most patients return home the same day and recover faster than after traditional vein surgery.

Nonthermal methods that use adhesive or medication are also available in selected cases.

Vein Surgery

Traditional ligation and stripping are used less frequently now because minimally invasive methods have replaced them for many patients.

Surgery still has a role when the anatomy is complex, when large surface veins cannot be treated with a catheter or when previous procedures have not solved the problem.

Removing or closing a faulty surface vein does not stop circulation. Deep veins carry most of the blood from the legs, and treatment redirects flow through working pathways.

Changes That Can Reduce Symptoms

Daily habits cannot reverse every form of CVI, but they can reduce pressure on the veins and slow progression.

  • Walk regularly
  • Avoid remaining seated or standing for several uninterrupted hours
  • Maintain a healthy weight
  • Stop smoking
  • Elevate the legs when resting
  • Use compression garments as directed
  • Keep the skin clean and moisturized
  • Limit excess sodium when swelling is a problem

The benefit of leg elevation is greatest when the ankles are above the level of the heart rather than simply placed on a footrest.

A person with a desk job can set reminders to walk for a few minutes each hour. Long-distance travelers should move their ankles, change position and walk when possible.

When Leg Symptoms Need Urgent Attention?

Chronic venous insufficiency develops gradually. Sudden symptoms may indicate a different and more urgent problem.

Seek immediate medical care for:

  • Sudden swelling in one leg
  • New calf pain, warmth or redness
  • Chest pain or unexplained shortness of breath
  • Coughing up blood
  • Heavy bleeding from a leg vein
  • Rapidly spreading redness around an ulcer
  • Fever with a painful or draining wound

Sudden one-sided swelling can indicate deep vein thrombosis. Chest pain or shortness of breath may mean a clot has traveled to the lungs.

Routine medical assessment is appropriate when swelling keeps returning, varicose veins become painful or ankle skin begins to darken, harden or itch.

An ulcer that does not heal also needs professional care. Venous ulcers can lead to infection and repeated tissue damage when the underlying pressure remains untreated.

What Patients Should Ask at an Appointment?

A useful evaluation should provide more than a diagnosis. Patients need to know what is causing the problem and which treatment fits the ultrasound findings.

Helpful questions include:

  • Do my symptoms come from superficial veins, deep veins or both?
  • Do I need an ultrasound?
  • Is compression safe for me?
  • What compression strength should I use?
  • Could a medication be contributing to the swelling?
  • Would a vein procedure improve my symptoms?
  • What should I do if the skin begins to break down?
  • Do I have signs of peripheral artery disease or lymphedema?

Patients with previous DVT should also ask whether post-thrombotic syndrome is contributing to their symptoms.

Living With Chronic Venous Insufficiency

CVI is generally a long-term condition, but symptoms and complications can be controlled.

Early treatment can limit swelling, protect the skin and reduce the risk of an ulcer. People with advanced disease may need ongoing compression and periodic follow-up even after a procedure.

Improvement is not always immediate. Skin discoloration may remain after vein pressure is reduced, and a longstanding ulcer can require months of care.

The most important step is recognizing that recurring swelling, heaviness and ankle skin changes are not simply unavoidable parts of aging.

A medical examination and duplex ultrasound can show what is happening inside the veins. From there, treatment can range from walking and compression to wound care or a minimally invasive procedure.
